A woman was killed on Sunday while skydiving at Lodi Parachute Center. According to KCRA, her parachute failed to deploy during her jump. This is the 16th death at the location since 1999. The FAA will investigate the incident, but said the diver was using her own equipment.

This comes on the heels of a recent FBI investigation of the Lodi Parachute Center. Earlier this year, agents served the center with a warrant. They did not release the findings, but left with multiple boxes and trash bags.

Additionally,  20 instructors at the center were suspended in 2016. 120 others were were made to undergo more training after an investigation by the United States Parachute Association.
